STUDENT APPLICATION PROCEDURES
An application is not a guarantee of admission. Students are admitted depending upon the number of vacancies that occur at each grade level. Pan American Academy Charter School does not discriminate against race, color, creed, or disability.
Step 1: Please print and fill out the application (Application pdf) or pick up an application packet at Congreso's main building.
Step 2: Gather all required documentation listed below (Document checklist pdf)

Immunization records
__ 4 DPT (one must be after 4th birthday)
__ 3 Oral Polio
__ 3 Hepatitis B
__ 2 MMR (both must be after 1st birthday)
__ 2 Varicella (both must be after 1st birthday) |

Copy of Social Security Card (if child has one) |
|
Completed Physical Form |
|
Picture ID (of parent or legal guardian) |
Step 3: Bring completed applications and supporting documents to Ms. Julie Roman or Ms. Linda Soto at Congreso's main building, Monday through Friday, from 8:00am to 3:00pm.
