PCM Care Coordinator

The PCM Care Coordinator is responsible for providing managerial and clinical oversight to Primary Client Managers, providing an interdisciplinary service approach for all welfare-to-work clients with other project components and the behavioral health services. The Coordinator will provide clinical support, coaching and mentoring to PCM's, facilitate weekly multi-disciplinary case conferences, establish operational guidelines for a One Family- One Plan approach to services and provide family care coordination and intervention.
Requirements: MSW required; CSW preferred; Minimum 3 years experience providing clinical services, Bilingual (English/Spanish)


Project Renaissance Director

Needed to manage daily operations of the center services to assist customers in obtaining their employment and training needs. Monitors program to ensure that performance and activities are compliant with contract requirements. Identifies and develops relationships with other relevant local government entities, community partners and business partners. Provide general oversight of the project to include coordination of staff work, scheduling, staff training and evaluations. Requirements: Bachelor’s degree in business, social work, human services or related field, required. MBA preferred. Minimum of five years of experience in a supervisory position, preferably working with at-risk adult population. Demonstrated knowledge of employment and training program administration, management and operation. Excellent oral and written communication skills. Ability to establish and maintain effective working relationships with supervisors, associates, outside agencies and the general public.
