Rapid Rehousing Stabilization Specialist

  • Position Summary

    The Rapid Re-Housing Stabilization Specialist reports to the Director of Family Services and is responsible providing housing for families’ in need of short-term rental assistance, financial assistance, and supportive services to households at risk of homelessness in eastern North Philadelphia. The Rapid Re-Housing Stabilization Specialist will also provide financial counseling services, negotiating terms of financial assistance with landlords and utility companies and assisting with housing relocation. This position will also provide basic case management and referrals to other organizations that provide supportive services or public benefits.

     

    The Rapid Rehousing Stabilization Specialist applies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee-focused experience that is appreciative of the unique strengths, values and needs that each employee brings to the workplace; and by using data to inform and drive decision making. Attention is given to compliance, quality assurance, and best practices as they relate to employee experience, records management, and data integrity and usability.

    Essential Functions

    Case Management/Compliance

    • Conduct initial client intake including needs assessment and the development of an action plan, regular follow-up with clients to measure and ensure goal attainment and client satisfaction.
    • Establish and maintain relationships with realtors, landlords, and financial institutions to locate accommodations for clients as needed.
    • Conduct home visits and home inspections, in as needed basis.
    • Provide a holistic approach by linking clients to services that can support self-sufficiency using PCM Model.
    • Assist clients to fill out Crisis, LIHEAP applications and to counsel them on any energy related issues.
    • Document each service provided in Congreso’s internal database and the city database, HMIS. Maintain client’s records and document all contacts with clients in the database.
    • Maintain accurate and up-to-date client files, documentation and electronic files.

    Administrative 

    • Develop statistical and narrative reports, in order to assist the program manager and/or program director in preparing reports or proposals to funders. Reports that are required by the Congreso are Rapid Matrix, Linkages, Rapid Tracker and any internal reporting requirements.
    • Conduct all phases of housing education/advocacy and counseling, under the program director’s supervision.
    • Participate, with the program manager and/or program director, in the development and/or refinement of data collection systems.
    • Regularly attend and actively participate in staff meetings, training sessions, case reviews, case consultations with medical case managers programs, etc.
    • Work closely with the Family and Housing Services Division, Education and Workforce Services Division, and the Health Promotion Wellness Division to create a pathway for client to achieve economic stability and independence.

    Minimum Experience, Education, and Qualifications

     

    Minimum Education: BA/BS degree from an accredited college or university with a field of study in Social Work, Psychology, Human Service, Education or related field required.

    Minimum Experience

    • 2 years direct experience in case management, human service, housing counseling with a focus of homelessness or equivalent setting or equivalent combination of education, training, and experience Knowledge of the Latino and urban community of Philadelphia.
    • Excellent oral and written communications skills; Bilingual (English/Spanish) preferred.
    • Computer proficiency in Microsoft Office Suite required (Word & Excel).
