Social Work Coordinator (temporary 3 month position effective 6/1/08)

The Alcanza Social Work Coordinator is responsible for 3 major functions: 1) Program Administration: the daily supervision of an Alcanza program case manager and all related tasks; 2) direct Service: intake, assess and provide home-based therapy for clients enrolled in the Alcanza Program. Co-serve up to 15-18 teens who are participating in the Alcanza Care Research Project intervention component, Facilitating therapeutic groups as needed. 3) Contract Compliance: Submit all data, quarterly and annual programmatic and statistical reports as required by the agency and funder guidelines. The Program coordinator will be directly supervised by the manager of the teen parenting services department. When the department manager is not available, the program coordinator will then be supervised by the Director. Requirements:Master’s degree in Social Work with one to two years work experience in individual or family counseling case management services; One to two years direct experience in the field of maternal and child health, public health or related field; Administrative, clinical and supervisory experience; Excellent oral and written communication skills in English and Spanish; Working knowledge of computers

Clinical Coordinator

The Prenatal Clinical Coordinator is responsible for two major functions: 1) Program Administration: the ongoing development and enhancement of programs and the daily supervision of the prenatal case manager of Un Buen Comienzo, and the external representation and marketing of programs; 2) Direct Service: facilitate intake and assessment process for clients who will be enrolled in the Un Buen Comienzo Program, co-serve 15-20 high-risk pregnant women who present with mental health concerns. The Clinical Coordinator will be administratively and clinically supervised by the Manager of Adult Maternal and Child Health Programs. When the manager is not available, the clinical coordinator will then be supervised by the Director of Family Health Programs. SAFE Program Client Manager

The Client Manager works under the direct supervision of the SAFE Program Manager. In the manager’s absence, the director of Health Promotion and Wellness, with the assistance of the Lead Social Worker, supervises the position. The position of Client Manager requires a high level of responsibility, integrity, and initiative. The Client Manager is responsible for the following: carry a caseload of moderate & high risk cases to provide support services; conducts outreach activities to recruit participants for home visiting services as needed, works with program manager and lead social worker with clients’ crisis intervention, assist clients with needed referrals for health care and social services, conduct home visits for program participants, provide education on maternal and child health & parenting skills, complete client data and programmatic and statistical reports. Requirements: Bachelor’s degree in Human Services, Social Work, or related field.Two to four years work experience in a human service, health care, community or educational setting; Knowledge of the Puerto Rican/Latino Community in Philadelphia; Excellent public speaking skills; Knowledge of pediatric/family health issues, insurance/medical assistance procedures with economical disadvantaged; Ability and willingness to conduct community outreach activities throughout the North Philadelphia community Bilingual/Bicultural (Spanish/English)

D&A Client Manager

The Client Manager position is supervised by the Client Management Coordinator of Drug & Alcohol Client management Services and in her absence the Director of Behavioral Health Services. The Client Manager will work closely with the other client managers and the Program Coordinator to assure quality services for all client management through Congreso’s integrated service delivery model, and appropriate documentation, contract compliance and reporting. Coordinate and advocate for a range of core and supportive services to address treatment related needs and education and/or employment. Requirements: Bachelor’s degree in Social Work, Human Services, Public Health or related field; Bilingual/Bicultural (Spanish/English) preferred; Knowledgeable about the Latino community in Philadelphia; Knowledgeable regarding Substance Abuse and its impact on the Latino community; Good oral and written communication skills; Ability to work under pressure and handle emergency situations; A valid drivers license.

HIV Prevention Case Manager

Needed to provide those living with HIV/AIDS intensive risk reduction counseling with the goal of minimizing the spread of HIV. Conducts and completes biopsychosocial history of clients. Provides ongoing client follow-up, risk reduction information and short-term emotional support. Requirements: Bachelor’s degree in Social Work, Counseling, Psychology or Nursing; Two years experience in counseling or social work with a preference on substance abuse or HIV infection services; Suitable combination of education and experience in related field will be considered in lieu of degree; Bilingual/Bicultural (Spanish/English); Ability to work under pressure and handle emergency situations; Sensitivity to the needs of persons with HIV infection a must; Good clinical skills and client record documentation skills; Knowledge of HIV/AIDS, drug and alcohol resources and services available within the Latino community

Alcanza Case Manager

The Case Manager works under the direct supervision of the Social Work Coordinator. In the supervisor’s absence, the Program Manager supervises the position. The position of Case Manager requires a sincere interest in working with and supporting the overall health and development of teen mothers, and their children and families. The Case Manager is responsible for working as a team with their supervisor to serve clients as follows: providing case management services, ongoing assessment, the development and/or review of service plans, responsibility for a caseload, documentation, referrals and follow-up, advocacy and submission of programmatic and statistical reports. HIV Case Manager

To provide those living with HIV/AIDS a comprehensive continuum of services through critical linkages with community resources.
